You must have original birth certificate from the Metropolitan Health, not the hospital. |

Registrations are done Monday - Friday from 9:00 am to 5:00 pm. |

Your registration are good for 1 month - no exceptions. If you registered and did not appear on the scheduled day, you will have to re-register again. The registrations will not carried over to the next month. Due to the high volume of paperwork and confusion this procedure is no longer performed. |

Registrations are 1 month prior to baptismal ceremonies. There are English and Spanish baptisms. The preference of the language is your decision. |

Only the mother or the father are allowed to register their child. |

You must live in the parish boundaries and show proof of residency. If you do not live in our boundaries you must be a registered parishioner. |

|
Sponsors (Godparents) should be married by the Catholic church. He/she should have received the sacrament of Baptism, First Communion, and Confirmation. Proof is required. |
| There is no charge for the Sacrament of Baptism, although a donation is appreciated. |

Completion of the pre-marriage questionnaire form with the priest. |

Review pre-marriage questionnaire results with the priest. |

Copies of baptism and confirmation certificates from the Catholic party dated within 6 months of the wedding. Baptized non-Catholics should also obtain a certificate or letter of proof of baptism. |

For an inter-faith marriage, a dispensation must be obtained by the Catholic party. Priest provides the form. |

Two affidavits for the bride and groom signed by two people well known to each person (parents or family members) for verification of freedom to marry. |

Attend either the Pre-Cana Programs or the Engaged Encounter Weekend. Priest will provide schedule of dates. |

If either the bride or the groom has been previously marred in a Catholic, Protestant, or civil ceremony, a declaration of nullity must be obtained. Priest provides form. |

|
Rehearsals are usually held on the evening before the wedding. The organist or music group does not need to attend rehearsal. The civil marriage license should be given to the at that time. |

|
Arrangements for the an organist, singer, or music group is made by the couple. If you provide your own organist, he/she must be capable and knowledgeable of our particular church organ. The priest can also supply you with a list of organists. |
